Medications
Parents/guardians, if your child requires medication during school hours, please ensure that you bring the following items to school or send them via fax or email to the school nurse:
- A medication order from your child's healthcare provider. A new order is required at the beginning of each school year. (Link to the form can be found here).
- Your signature on the medication order form, authorizing the school to administer the medication as directed.
- The medication must be brought to school by an adult in a properly labeled prescription bottle.
- It is important to ensure that your child understands how and when to take their medication.
Life-Threatening Allergies
This MCPS website page offers detailed information regarding anaphylaxis management for students with allergies requiring epinephrine.
In order for students to receive appropriate treatment,
- An Anaphylaxis Action Plan must be completed and signed by their healthcare provider. This plan is valid for one year from the date of signing. (Link to form can be found on this page)
- Bring the prescribed medication to the school.
- Depending on the student's age and level of development, they may be allowed to carry and/or self-administer their epinephrine, subject to approval by their parent, healthcare provider, and school nurse.
- It is important to review the Parent and Student Anaphylaxis Checklist with your student to ensure a smooth start to the school year. (The checklist link can be found here).
Asthma
This MCPS website page offers detailed information regarding asthma management at school.
In order for students to receive appropriate treatment,
- An Asthma Permission Form must be completed and signed by their healthcare provider. This plan is valid for one year from the date of signing. (Link to form can be found on this page)
- Bring the prescribed medication to the school. Please contact the school nurse if an inhaler needs to travel between home(s) and school.
- Depending on the student's age and level of development, they may be allowed to carry and/or self-administer their inhaler, subject to approval by their parent, healthcare provider, and school nurse.
Diabetes
Extensive information on managing diabetes at school can be found here. Please contact me, the school nurse, if your student has a new diagnosis of diabetes or when changes are made to their care.
In order for your student to receive appropriate care,
- A current Diabetes Medical Management Plan (DMMP) completed and signed by their healthcare provider must be provided at the beginning of each school year, and whenever changes are made. (Link can be found on this page).
- Bring all needed supplies to the school at the beginning of the school year, including treatment for hypoglycemia ("lows"), back-up insulin administration supplies (sensors, tubing, etc.), ketone sticks, glucometer and test strips, lancets, etc.
- Review the Checklist for Parents of Students with Diabetes to ensure a smooth start to the school year.
Seizures
This MCPS website page offers detailed information regarding seizure management at schools.
In order for your student to receive appropriate care at school,
- A Seizure Action Plan completed and signed by their healthcare provider must be provided. A new order is required annually. (Link found on this page).
- The parent/guardian must bring the prescribed medication to the school in a pharmacy-labeled container.

Immunizations
Refer to the MCPS Health Services webpage for information on immunization requirements for school. Turn in updated immunization paperwork to the school secretary.
