Please complete the information listed below and give it to each person you have asked to write a letter of recommendation for you. Submit your request to the teacher at least two weeks before the application deadline.  Be sure to remind the teacher of the deadline for recommendations to be completed.  

Remember that your counselor is generally required to submit a letter of recommendation with a college application School Report, so your counselor would need the information listed below as well. Note that if you have an application deadline that falls during Christmas break, you must request the recommendation by early December. Be sure to send a Thank You note to the writer. Good Luck!
