1095-C Statement Available Upon Request (Calendar Year 2025)
A 1095-C is a tax form employers use to report health coverage offered to employees. It is part of the Affordable Care Act requirements.
This form is not required to prepare or submit with your taxes. However, it helps employees determine whether their employer offered them coverage that meets ACA standards when filing taxes.
The Paperwork Burden Reduction Act recently passed, now provides flexibility in terms of how employers can provide 1095-C forms to employees. Employers are no longer required to mail the forms.
